The Doctor completes the application and provides supporting information ie a copy of the Australian Health Practitioner Regulation Agency registration (which must show that the registration is current and unconditional) and Certificate of Currency of Medical Indemnity Insurance including a receipt of payment together with the name of the insurer, and the amount insured for. The application form will outline additional information required.

On receipt of the completed application form and associated evidence, the application will be reviewed by Perth Clinic’s Credentialing Committee who will make a final recommendation to the Medical Advisory Committee. Once ratified by the Chief Executive Officer you will be notified of the outcome of your application.
